Brukermedvirkning og samvalg i døgnbehandling av ungdom med psykisk lidelse

Bakgrunn: Psykiske helseplager er beskrevet som en av de største utfordringene for folkehelsa og framtiden, og rammer i særskilt grad ungdom. Lovgivning og forskningslitteratur understreker behovet for at behandling vektlegger brukermedvirkning, og samvalg er spesielt anbefalt som metode. Samvalg kan betraktes som en prosess med høy grad av brukermedvirkning som finner sted i et partnerskap mellom pasient og helsepersonell. Ved samvalg skal ungdom delta i behandlingsavgjørelser, og individuelle mål, verdier og behov vektlegges. Offentlige rapporter viser at vi har en lang vei å gå med å tilpasse helsetjenestene til ungdommenes behov, spesielt for de med sammensatte behov og alvorlig psykisk lidelse. Det er lite tidligere forskning som har undersøkt brukermedvirkning og samvalg i døgnbehandling av ungdom med psykiske lidelser.
Mål: Denne avhandlingen tar for seg døgnbehandling av ungdom med psykiske lidelser, og har som mål å undersøke erfaringer med brukermedvirkning og samvalg og forhold som påvirker gjennomføringen av dette. På bakgrunn av perspektivene til helsepersonell, ungdom og foreldre utforsker avhandlingen hva samvalg er i konteksten psykiske helsevern for ungdom, samt muligheter og utfordringer knyttet til både brukermedvirkning og samvalg.
Metode: For å undersøke erfaringer med brukermedvirkning og samvalg ble det benyttet kvalitativ metode med et eksplorerende og induktivt studiedesign. Avhandlingen ble designet i samarbeid med to medforskere som hadde erfaring fra helsetjenestene. I tillegg bidro et panel med ungdom med brukermedvirkning i forskningen. Det ble gjennomført tre delstudier med henholdsvis fokusgruppeintervjuer med helsepersonell (delstudie 1) og individuelle intervjuer med ungdom (delstudie 2) og foreldre (delstudie 3). Utvalget bestod av totalt 37 informanter som ble rekruttert gjennom psykisk helsevern for ungdom ved to sykehus og et nasjonalt prosjekt i regi av Forandringsfabrikken som inkluderte flere helseforetak. Dataanalyser inkluderte systematisk tekstkondensering (delstudie 1), tematiske innholdsanalyser (delstudie 2 og 3), samt en kvalitativ metasyntese som identifiserte mønster på tvers av de tre delstudiene.
Resultat: Delstudie 1 utforsket helsepersonell sine erfaringer med brukermedvirkning og samvalg. Deltakerne vektla et behov for endringer i kultur og rutiner ved klinikkene for å oppnå individualiserte tjenester tilpasset den enkelte ungdom sitt behov. Brukermedvirkning og samvalg forutsetter at ungdom møtes med omsorg, og det krever tid å bygge en trygg relasjon. Korte innleggelser og standardiserte behandlingsforløp utfordrer brukermedvirkning og samvalg og stiller krav til fleksibilitet blant helsepersonell. Deltakerne hadde erfart at dialog med ungdommene og brukermedvirkning bør starte forut for innleggelse, og samvalg forutsetter at møter der beslutninger tas tilrettelegges for ungdom sin deltakelse.
Delstudie 2 utforsket ungdom sine erfaringer. Resultatene viste at informasjon og etablering av en tillitsfull relasjon med behandler var en forutsetning for brukermedvirkning og samvalg. Ungdom som ikke opplevde å bli lyttet til og ha medbestemmelse ved innleggelse uttrykte motstand eller likegyldighet til behandlingen. Diagnoser ble opplevd som stigmatiserende og begrensende for individuell tilpasning, valgmuligheter og samvalg. Utforming av en behandlingsplan som passet for den enkelte ungdom, og som ble fulgt opp også etter utskrivning, ble vektlagt for at brukermedvirkning og samvalg skulle ha en reell funksjon.
Delstudie 3 utforsket foreldre sine erfaringer. Resultatene viste at forelde stod i et dilemma; De ønsket brukermedvirkning og størst mulig grad av selvbestemmelse for ungdommene, samtidig som de så at foreldreinvolvering var nødvendig for etablering og oppfølgning av helsehjelp. Foreldrene opplevde å ha viktige roller i samvalg uten at de var beslutningstakere. De formidlet informasjon og bidro med nødvendig støtte til ungdommene, men vektla at de selv hadde behov for familieorienterte tiltak for å holde ut. Ungdommene sitt behov for individualisert behandling og kontinuitet ble vektlagt. Foreldre opplevde imidlertid at det stilte store krav til dem som pådrivere og koordinatorer.
Metasyntese av de tre delstudiene resulterte i fem tverrgående tema som var viktige for brukermedvirkning og samvalg: 1) anerkjenne og balansere medvirkning; 2) relasjon og tillit; 3) informasjon og opplevd kontroll; 4) samarbeid og koordinering; og 5) personsentrert fremfor diagnosesentrert.
Diskusjon og konklusjon: Avhandlingen gir en dypere forståelse for kompleksiteten ved brukermedvirkning og samvalg i døgnbehandling av ungdom med alvorlig psykiske lidelse. Behovet for balansering av ungdom, helsepersonell og foreldre sin sine perspektiv og behov demonstreres. Muligheter og utfordringer med å engasjere ungdom i egen behandling og samarbeid mellom involverte aktører forstås i lys av teori, og det presenteres en modell for medbestemmelse og samvalg
Redusert sykdomsinnsikt ved alvorlig psykisk lidelse, mulig uenighet mellom ungdom, foreldre og helsepersonell, samt mange aktører i behandlingskjeden utfordrer ungdom sine muligheter for brukermedvirkning i døgnbehandling. Brukermedvirkning og samvalg er utfordrende når konteksten er korte innleggelser med standardiserte behandlingsforløp ved akuttenheter som er presset på kapasitet. Samtidig har brukermedvirkning og samvalg potensiale til å øke ungdom sitt eierskap til egen behandling, og til å redusere reinnleggelser og behov for tvang. Det forutsetter en tilnærming som vektlegger trygge relasjoner mellom ungdom og helsepersonell, informasjon, tilstrekkelig tid, fleksibilitet og kontinuitet i behandlingen. En dialog med ungdom om brukermedvirkning og samvalg bør fortrinnsvis starte før innleggelse. Foreldre innehar viktige roller for gjennomføring av samvalg men har selv behov for informasjon og støtte. Ulik forståelse blant fagfolk om hva ungdommene sine problemer og behov er, hemmer kontinuitet og understreker behovet for koordinering av helsetjenestene.
Brukermedvirkning bidrar til å redusere en asymmetrisk maktbalanse mellom ungdom og helsepersonell. Samvalg gir oss en modell for hvordan reell brukermedvirkning i behandlingsavgjørelser kan gjennomføres, men forutsetter målrettede tiltak i helsetjenestene. Avhandlingen anbefaler en personsentrert tilnærming i døgnbehandling av ungdom med psykiske lidelser. Behandling som kombineres med familieorienterte tiltak ivaretar foreldre og utnytter samtidig den ressursen de representerer.
